Certificate of Registry a.k.a. Pink Slip

What is a Pink Slip? A pink slip as it is commonly called is a Certificate of Registry that is given to the parent(s) or guardian of a new born or to a person who has been registered by the process of a Late Registration or Re- Registration.
 A Certificate of Registry is not a birth certificate it contains vital information such as the birth entry number, child’s name, parent’s name or in the case of a single parent mother’s name, date and place of birth and date of registration. The Certificate of Registry is given to the parent(s) or guardian as a receipt after registration.   A fee of Two Hundred dollars ($200.00)  is paid for the registration or if it is a late registration/re-registration a fee of Three Hundred dollar ($300.00) is paid to the Registration Officer/LDR.
The Certificate of Registry is used when applying for copies of birth certificate; it is also used to verify the correct spelling of name or names that might have entered on the original record incorrectly.
