Supporting Documents for Late Entry of Name
The Registration Births and Deaths Act, provides for every child that is born in Jamaica and Jamaican waters and airspace to be fully registered, including with a name. In cases where a child was registered without a name it would be a hospital birth where the parents did not attend the registrar’s office and complete the registration. If the child’s registration did not include a name after twelve months, a Late Entry of Name (LEN) will be advised.
This is done by completing the Late Entry of Name form which expects an authorized person, primarily the parents, to make the declaration of the child’s name. (The LEN forms can be printed from the Agency’s website at www.rgd.gov.jm)
Persons over the age of 65 years are able to declare for themselves, and as such the following documents are required:
· Copy of marriage (if married)
· Child/children birth certificate(s)
· Valid form of identification (National ID, passport, health card, Deed Poll etc.)
· In the absence of a school record internal evidence such as: Marriage, children’s birth and death certificate will be sufficient to support the amendment to the record
Please note that any other documents bearing complete name can be used as evidence.
For children 12 years and under the following documents are required:
· Mother’s ID
· Immunization Card
· Baptismal certificate
In a case where the mother cannot be found and the father is conducting the Late Entry of Name for the child, the father must add his particulars on the child’s birth record, hence a Status and the Late Entry of Name must be processed at the same time.
If the father or mother is not available to conduct the Late Entry of Name the guardian must write a letter stating this and have that letter signed by a Justice of the Peace, if overseas, by a Notary Public.
For persons who do not fall in these two categories, please note that All sections of the Late Entry of Name form MUST be accurately completed.
